THE BENEFIT ENROLLMENT CYCLE
The health & welfare benefit plans run from November 1st to October 31st of the following year (including two benefit periods from Nov 1 to April 30, and from May 1 to Oct 31).
You must re-enroll every year. Before enrollment begins on September 24, 2009, enrollment packets will be mailed to all eligible graduate employee's departmental mailboxes. The enrollment packet can also be downloaded on this website. These packets are for information purposes only: enrollments are done on-line using the following simple steps:
HOW TO APPLY
1) Go to http://hwtrust.geouaw.org
2) Fill out your personal information and choose your plans (you will need your Social Security Number (SSN) and a UMass Student ID number, and if you are enrolling dependents, you will need their date of birth). Remember to write down the user name and login you create for your account.

3) Confirm your enrollment
4) Print & sign your authorization form, and bring it to GEO (201 Student Union). If you are choosing the family dental plan, payment of the $100 family dental plan premium is due with your authorization form.
The authorization form allows us to verify your eligibility for benefits. We cannot enroll you without one.
You
will receive an email confirming your enrollment within approximately 2
weeks of completing your application. Please plan accordingly.
DEADLINES
The priority enrollment deadline for plan year 2009-10 is Wednesday, October 7th at NOON. You must apply online and return your signed enrollment authorization form by this deadline in order to guarantee November 1st coverage. We cannot guarantee against gaps in your coverage if your application is received later than this.
If you are enrolling for family coverage, and wish to pay your premium via payroll deduction, you must return your payroll deduction form by the same deadline.
